Entry Forms
Entry forms for Visual Art Exhibits may be downloaded and printed. Two (2) copies of the entry forms are needed for each entry. Attach one copy to the back of a two-dimensional work or to an unobtrusive area of a three-dimensional work. The second copy should be submitted with the work at drop-off.
- Download the Visual Arts Entry form. It is a PDF document compressed into a ZIP archive.
- Unzip the file.
- Open the PDF form in Adobe Reader.
- Complete one section of the form for each work being entered. The form can be filled out using your computer. (Alternatively, you can print the blank form and enter the information by hand. Please print using a pen.)
- Print three copies of the document. Two copies will be submitted with each work being entered. The third copy is for your records.
If available, copy your Festival Identification Number (MFoA ID) found on your mailing label onto your entry forms in the spaces provided. Note: New artists usually will not have an MFoA ID. If you are a returning artist, you may request your MFoA ID:

Eligibility: Persons who reside, work, or
belong to an art association in Essex County. (For Student and Youth Art eligibility requirements, see Student Art or Youth Art.)
Exhibit: Accepted work will be displayed
during the Festival and may not be removed until 5:00 PM on
the last day. No exceptions will be made.
Sales: Artists may offer their works for
sale. All work so offered must be sold as displayed. The Marblehead
Festival of Arts, Inc., will receive a 20% commission on all
sales.
Pick Up Accepted Work: Accepted work shall
be picked up after the Festival during the specified times.
Artwork not picked up within 60 days after the close of the
Festival shall become the property of the Marblehead Festival
of Arts, Inc.
Liability: The Marblehead Festival of Arts,
Inc. (MFoA) is not responsible in any way for the loss, damage,
disappearance, or harm of any kind to the work of art. Delivery
of works to the MFA constitutes an express waiver, release,
and discharge of all claims, actions, or causes of action
against the MFoA for harm of any kind to the work of art.
Entry Form:Each work must be accompanied
by a completed Entry Form.
Entry Fee: Each work must be accompanied by the appropriate
Entry Fee, where applicable.

All students in Marblehead public and private
schools, and in Marblehead art studio classes, must submit works
through their teachers. Marblehead resident students who attend
schools or art studio classes outside of Marblehead must drop
off their own entries:

Due to exhibit space limitations, we are unable to accept many fine works for exhibition. Postcards will be mailed to artists indicating whether works are accepted or not accepted.
